SkateSafe / SafeSport guidelines require that all electronic communication regarding minors must include an adult 18 or older. This is the person that will be contacted for billing and all important communications regarding the class.

If all skaters are registering for the same sessions, enter all skaters on one form. Otherwise, please fill out one form per skater.
Please list the name(s) and birthdate(s) of the skater(s) planning to participate. (Do not list any adults here.) Date of birth will be used to best assign skater to an appropriate group.
Coaching Client Policies
General Agreement
By working with Grace & Light Ice Dance, LLC dba WeGlīd and it's employees, herein ‘the Coach’, the skater and his or her parents or guardians and anyone designated as a primary contact or financially responsible individual, herein ‘the Client’, agree to the following terms. All changes to the terms will be communicated to the Client in writing at the primary contact e-mail address provided at registration at least 7 days before the updated terms of service go into effect.
*Grace & Light Ice Dance, LLC dba WeGlīd, Ice Zone Inc. dba Prince William Ice Center, and Haymarket IcePlex, are independent contractors. They and their affiliates are not employees, general agents, subsidiaries, joint venturers, partners or servants of each other.

Cancellation Policy
Cancellation by Client
Prompt Communication Requested: If the Client must cancel, the Client agrees to notify the Coach as early as possible. At the discretion of the Coach, frequent no-shows or late cancellations may result in loss or change of lesson slot.
No Make-ups, Refunds, or Pro-rating: If the Client does not attend a lesson, the Client will be charged for the full lesson. No refunds will be issued.
Cancellation by Coach
Method of Communication: To communicate any cancellations or to reschedule, the coach will contact the Client at the primary contact number and e-mail provided by the client.
Refund Policy
A refund will only be given in the event that the coach is unable to provide a lesson at the agreed upon time or another time that is acceptable to both the Client and the Coach. No other refunds will be given.

The Coach will strive to resolve any issues to the best of his ability. To ensure that the Client is content, it is necessary that the Client communicate any and all complaints and questions, whether related to lessons or billing, directly to the Coach immediately in writing at [email protected]

The Client will strive to help resolve any issues. The Coach will communicate directly with the Client all complaints and questions, whether related to lessons, billing, or student behavior issues.
Physical Contact Policy
As explained in the SkateSafe handbook, “Physical contact in sport is inherent in instruction and training. Appropriate physical contact has two elements: (1) The physical contact takes place in a public area where interactions can be easily observed and at an interruptible distance from another Adult. There is no potential for physical or sexual intimacies during the physical contact. (2) The physical contact is for the benefit of the Minor Athlete or Participant, not to meet an emotional or other need of an Adult Participant.”
Coach Lucas will ask for verbal permission from a skater and explain the action before initiating any physical contact. The skater should feel empowered to give consent, ask questions, or deny consent for any reason at any time, and recognize that this request for permission is always a real question. Consent can be revoked by the child at any time. The parents of a minor should ensure that their child knows it is expected and okay to deny consent if the child feels uncomfortable at all. In cases of partnered ice dance, it is understood that physical contact is necessary in terms of normal dance positions and lifts.

PARTICIPANT WAIVER AND RELEASE OF LIABILITY — ASSUMPTION OF RISK AND INDEMNITY AGREEMENT
Waiver Form for MINORS (Under Age 18)
Event/Activity: Ice Skating — Lessons with Grace & Light Ice Dance, LLC dba WeGlīd
Date(s): July 1, 2024 through June 30, 2025
Location(s): Prince William Ice Center (Woodbridge, VA) and Haymarket IcePlex (Haymarket, VA)
For and in consideration of Grace & Light Ice Dance, LLC dba WeGlīd and all employees and contractors (“Instructor/Coach”) allowing the minor identified below (“Minor”) to participate in the event or activity described above (the “Event” or “Events”); I, for myself, and on behalf of Minor, and the Minor’s parents/legal guardians, heirs and next of kin, and any legal and personal representatives, executors, administrators, successors and assigns, hereby agree to and make the following contractual representations pursuant to this Waiver and Release of Liability, Assumption of Risk and Indemnity Agreement (the “Agreement”);
I hereby represent that (i) I am the parent or legal guardian of the Minor; (ii) the Minor is in good health and in proper physical condition to participate in the Event; and (iii) the Minor is not under the influence of alcohol or any illicit or prescription drugs which would in any way impair the Minor’s ability to safely participate in the Event, and that I am responsible for the Minor’s safety and well being at all times and under all circumstances while at the Event.
I understand and acknowledge the risks and dangers associated with the Minor’s participation in the Event and the sport of figure skating and related figure skating sport disciplines, including without limitation, the potential for serious bodily injury, sickness and disease, permanent disability, paralysis and loss of life; loss of or damage to equipment/property; exposure to extreme conditions and circumstances; contact with other participants, spectators, or objects; dangers arising from imperfect conditions; water and surface hazards; equipment failure; inadequate safety measures; participants of varying skill levels; situations beyond the immediate control of the Event Organizers; and other undefined, not readily foreseeable and presently unknown risks and dangers (“Risks”). I understand that these Risks may be caused in whole or in part by the Minor’s own actions or inactions, the actions or inactions of others participating in the Event, or the negligent acts or omissions of the Released Parties defined below, and on behalf of the Minor, I hereby expressly assume all such Risks and responsibility for any damages, liabilities, losses or expenses which I incur as a result of the Minor’s participation in any Event.
I understand that the Minor is required to be familiar with and to abide by the Rules and Regulations established for the benefit and safety of all participants in the Event. I accept sole responsibility for the conduct and actions of the Minor while he or she is participating in the Event, and the condition and adequacy of the Minor’s equipment. I understand that the Event Organizers have the right to disqualify anyone or deny participation in the Event for any reason they deem just and proper in their sole discretion.
On behalf of the Minor, I hereby Release, Waive and Covenant Not to Sue, and further agree to Indemnify, Defend and Hold Harmless the following parties: Grace & Light Ice Dance, LLC dba WeGlīd, Lucas Marquardt, Ice Zone Inc. dba Prince William Ice Center, and Haymarket IcePlex (Individually and Collectively, the “Instructor/Coach” or “Released Parties”), with respect to any liability, claim(s), demand(s), cause(s) of action, damage(s), loss or expense (including court costs and reasonable attorneys fees) of any kind or nature (“Liability”) which may arise out of, result from, or relate in any way to the Minor’s participation in the Event, including claims for Liability caused in whole or in part by the negligent acts or omissions of the Released Parties. I further agree that if, despite this Agreement, the Minor, or anyone on the Minor’s behalf, makes a claim for Liability against any of the Released Parties, I will indemnify, defend and hold harmless each of the Released Parties from any such Liabilities which any may be incurred as the result of such claim.
I hereby warrant that I am of legal age and authorized to enter into this Agreement on behalf of the Minor, that I have read this Agreement carefully, understand its terms and conditions, acknowledge that I will be giving up substantial legal rights by signing it (including the rights of the Minor, the Minor’s parents/legal guardians, heirs and next of kin, and any legal and personal representatives, executors, administrators, successors and assigns), acknowledge that I have signed this Agreement without any inducement, assurance or guarantee, and intend for my signature to serve as confirmation of my complete and unconditional acceptance of the terms, conditions and provisions of this Agreement. This Agreement represents the complete understanding between the parties regarding these issues and no oral representations, statements or inducements have been made apart from this Agreement. If any provision of this Agreement is held to be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this Agreement and shall not affect the validity and enforceability of any remaining provisions.
Photo/Video Consent and Release
I, the undersigned, do hereby release all rights or claims in connection with the photos/videos in which I appear, for use by Grace & Light Ice Dance, LLC dba WeGlīd. I understand the photo(s)/video(s), may be used for the promotional and educational purposes. I agree to the use of my name, likeness, photo and voice for educational, program publicity, website and organizational promotional purposes. I waive any right to inspect or approve the finished copy. I agree that the photos/videos may be broadcast and distributed without limitation through any means and I shall not receive any compensation or royalties for my participation.
I further release Grace & Light Ice Dance, LLC dba WeGlīd, its employees and agents from all liability which may arise from any or all claims by me or any third party in connection with my participation in the photo(s)/video(s). All photos and videos shall constitute the sole property of Grace & Light Ice Dance, LLC dba WeGlīd.
I declare that I am of legal age and have every right to contract in my own name in the above regard, and I am freely, knowingly and voluntarily signing.
If minor children are photographed or videoed, I declare that I am the legal guardian of these minor children and permit the use of these photo(s)/video(s) with the minor children by Grace & Light Ice Dance, LLC dba WeGlīd in accordance with the consent and release agreement stated above.
